COMPLETE HEALTH ASSESSMENT
You’ll need to take the private health survey the same day as your purchase. A licensed provider will review your answers to determine eligibility1. If eligible, you’ll receive a Letter of Medical Necessity (LMN) within 1–2 days.
SUBMIT FOR REIMBURSEMENT
Follow the guide in your LMN to submit your purchase for reimbursement2. Claims are usually paid out in 2–4 weeks.
